Eligibility

• The candidate must have passed 10+2 or equivalent examination with Any stream from CHSE, odisha or any other board or council or University equivalent thereof.
• Candidate shall be medically fit for the course.
• Both female/ male candidates are eligible to apply.

Documents to be submitted : -

• The intending candidates must submit their application in the prescribed form to the Administrative Officer, Gitanjali School of Nursing, Bhadrak.
• Xerox copies of the following documents should be enclosed with the application form.
• Mark sheet of Secondary school leaving certificate and +2 (or) Equivalent.
• 4 copies of duly attested passport size photographs.
• Conduct certificate from head of the institution, wherein the applicant studied for his/her qualifying examination.
• Transfer Certificate / College Leaving Certificate
• Xerox Copies Adhar Card
• Caste Certificate (For SC/ST)
• Income Certificate
• Residence Certificate
